MENTAL HEALTH SKILL BUILDING SERVICES
Services that are provided to individuals 18 years and older who suffer from serious mental illness, mental retardation or substance abuse.
An individualized service plan is created to identify the basic needs of the consumer to assist them in accessing proper services and supports that are essential to their everyday means of living.
This home and community-based program includes life skills training, individual and family counseling, anger management, and case management. We provide these services to individuals with a mental health diagnosis and who meet the eligility criteria.

Eligibity Criteria

Individuals must have an Axis I DSM diagnosis that results in significant functional impairments in major life activities. Or any other Axis I mental health disorder that a physician has documented specific to the identified individual within past year. Additionally individuals must meet at least two of the following criteria:

Have a prior history of psychiatric hospitalization, residential crisis stabilization, ICT, PACT, RTC level C, or TDO.

Exhibit inappropriate behavior that requires repeated interventions by the mental health, social services, or judicial systems.

Prescribed medication for anti-psychotic, mood stabilizing or anti-depressant medications within the 12 months prior to the assessment date.

Require help in basic living skills, such as maintaining personal hygiene, preparing food and maintaining adequate nutrition, or managing finances to such a degree that health or safety is jeopardized.

Individuals under the age of 21 shall be in an independent living situation or actively transitioning into an independent living situation.
